In the event of student absence, the parent/guardian is asked to call the school and leave a message.

Medical Conditions

Please inform the school if your child has any medical condition and/or requires medications while at school. If a child needs any prescriptions dispensed at school there must be a doctor’s signed Student Health Plan form on file at the school. You can pick up this form at the school office or on the CBE Forms You Need page.

Emergency Contact Information

We ask that you keep your address, phone number and emergency contact information at the school current by contacting the office at any time to notify us of any changes.

Illness and Injury

Sick children who are sent to school increase the likelihood that colds, flu, and other diseases will be spread throughout the school community. If your child is sick please keep him/her at home until well enough to resume full day activities at school, including outdoor recess and physical education classes. Children who are healthy enough to attend school are expected to go outdoors during recess and noon breaks. Please send a note with your child if he/she is unable to go outdoors due to a recuperation period following an extended illness.

Severe Allergies

As a caring school community, we must be aware and vigilant around the variety of severe allergies that many of our students have, the most common one being peanut and nut allergies.

We kindly request all families to support the common good of all by not sending any peanut/nut products to school. Students are responsible for their own daily snacks and are asked not to share food. We thank you for sharing the responsibility for the safety of all of our children.

Parents of students who have a severe allergy should complete a Student Health Emergency Response Protocol form (to be completed annually). It is our procedure at W. O. Mitchell School to ask parents at the beginning of the school year to provide the office with an extra epi-pen, asthma medication, or any other medication that your child(ren) may need in the event of an emergency.
