The following forms will be sent to parents/guardians by email in September or are available at the
Forms You Need page on the CBE website. Please download transportation, noon supervision and waiver forms online.
Forms may also be picked up at the school office.

- School District Use of Personal Information (for information only - yours to keep)
- Acceptable Use Policy for Technological Devices and Issuance Receipt
- Annual Volunteer Registration Form & ePIC Volunteer information
- *Medical Forms – Student Health Plan
- *Student Health Emergency Response Protocol Form
All forms need to be returned by late September.
*Return forms only if required by your children.
