Forms You Need 

Student Health Emergency Response Protocol Form
This form is to be used when there is a student health issue regarding attention. This includes when there is a request to administer medication. 

Student Health Plan

This form is to be:

  • completed if a physical or medical condition may affect the student's attendance at school
  • completed if medication is to be taken at school,
  • reviewed and updated annually or sooner if there is a change in the student's health concern or school registration.

Medical information that is shared with the school is used in an emergency or to support students if the medical condition impacts learning. Providing the school with this information allows the school to support your child to the best of their ability. If there is a serious medical condition that impacts learning, please arrange a meeting with your child’s teacher,
Student Services, or administration to review your child’s needs. 

All students who have a medical condition that could impact learning or could compromise their safety needs to have a Student Health Plan completed by their parent/guardian. Medications are securely stored in the office, and staff is made aware when necessary after consultation with the parent. This form is completed annually and medication kept up to
date.
