Forms Required

On the first day of school, your child will receive two packages of forms and a Demographic Verification Form.  The white package of forms is for your information and does not need to be returned.  The coloured package of forms along with the Demographic Verification Form need to be completed by you and returned to the school no later than September 10, 2021.  Please do not separate these forms but keep them together as a package so that none are lost.
